I acknowledge receiving a C2G Environmental Consultants time and materials rate schedule and authorize C2G to provide response services. I understand and accept the pricing and terms of the rate schedule. Invoices will be presented on a timely basis in accordance with the proposal and/or time and materials rate schedule provided. The Company or Individual agrees to pay all balances due upon receipt.

This agreement is between the Company or Individual signing below and C2G Environmental Consultants. If the Company or Individual seeks compensation from their insurance company or other Indemnity organization, C2G does not release the Company or Individual from their obligations under this agreement. Any deposit received by the individual signing this form will be held until full payment is received by the insurance company.

I understand that a finance charge of 1 ½% per month will be charged on all past due accounts. I will be responsible for all collection costs on past due accounts, including attorney’s fees.
